Line 1: It wants your last and first name in japanese characters (kana or kanji will do fine, make something up)
Line 2: It wants your last and first name in kana now
Line 3: Phone number
Line 4: Address
Line 5: Phone number
Line 6: Date of birth, in order YYYY/MM/DD (you have to be over 21, so if you have to, make it up)
Line 7: Male or female

The examples there should do, for the most part

Wasn't one of them hiragana?

The first asks for your name in kanji and the second asks for katakana, but just says kana next to the boxes. Katakana & Hiragana = kana, so you could write it in hiragana if you felt like it, I guess.

Line three is for your zip code (or house number?) not telephone.

Yeah, line 3 is zip code, I made a rather big typo, clearly

the hard way:
try to find real info to fill in, and download a program so you can type JP.
the easy way:
you will find a example underneath each input field, just copy and paste the text behind the : into the inputbox and you should be fine.

i used this alot to go on BB jp. works just fine, ust cant go over Lv20 and you have to make a new account after 15 days

to sign up, I just copied the example entries they gave. I have the Japanese IME, but I'm too lazy to fit the format they want the addresses in. boooo work. Also, I found playing to level 20 entirely pointless and unfun.
